Disease Risk Management Programme
The aim of the Disease Risk Management Programme offered by LMS is to improve your quality of life by helping you maintain a healthy lifestyle. This is achieved by providing you with education, advice and support from qualified nursing case managers.

Conditions include, but are not limited to: Asthma, Chronic Obstructive Pulmonary Disease, Diabetes, Hypertension, Cardiac Disease, Epilepsy and Bipolar Mood Disorder.

Eligible members will be invited to join the programme. Usually members whose conditions are already controlled are not contacted. If you are on a risk management programme, make sure you maximise the benefits of the programme by participating actively – reading the info leaflets and making positive lifestyle changes.

Treatment Plans
Are you a member who is registered for chronic medication for one of the 27 Chronic Disease List (CDL) conditions? If so, you will receive a unique treatment plan that lists services you can access to assist in managing your chronic illness. These services are covered from the insured (Major Medical) benefits and may include consultations with appropriate medical specialists and certain relevant pathology tests among others.
Smoking Cessation Programme
Most smokers want to quit smoking. If this is you, let LMS help you. Contact our Disease Management Department to register for the smoking cessation programme. You’ll then be contacted telephonically on a monthly basis for a specified period, and given info to guide you along the path to better, smoke–free health.

Preventative Care Cover
Good health is not just about feeling great – in the real world, it’s also about saving time, money and stress. For this reason, Liberty Medical Scheme covers a variety of preventative procedures and vaccinations to help guard against disease and keep your health on track so you can enjoy quality of life.

HIV/Aids Programme
